This is a reminder that you are responsible to upload your updated Driver’s License as well as Insurance Documentation BEFORE it expires. You will not be able to work without this valid documentation
Higher Level Insurance Required Documents:
Please provide a copy of your declaration pages that also shows the following: Effective Date, Expiration Date, Insured Drivers (Your Name), Vehicle Information, Policy Number, and Level of Coverage. Usually, the declaration page consists of 1-2 pages, sometimes 3 pages depending on the number of drivers, vehicles, and coverage you are carrying.
Lower-Level Insurance Required Documents:
Please provide a copy of your insurance card that also shows the following: Effective Date, Expiration Date, Insured Drivers (Your Name), Vehicle Information, and Policy Number.
If you could upload it to your ESS at your earliest convenience, I would greatly appreciate it.
